May 8, 2002 - Trial Court Approval

May 15, 2002 - Opt-Out Deadline

June 7, 2002 - Final Judicial Approval

Claim Form Submission Deadlines

Orange Form - Class Members who choose the Orange Form and DO NOT choose the Guaranteed Payment Option must submit this completed form, postmarked no later than 180 days after Trial Court Approval or 180 days after an Affected Product Revision Surgery.

Class Members who choose the Orange Form and DO choose the Guaranteed Payment Option must submit this completed form, postmarked no later than 120 days after Trial Court Approval or 120 days after an Affected Product Revision Surgery.

Blue Form - September 5, 2002

Green Form - Must be submitted along with a timely Blue or Orange Form (whichever is appropriate) and postmarked on or before the date that is the later of:

• 545 days after the date of applicable Covered Revision Surgery; or
• 180 days after a Non-Removal Surgery; or
• 180 days after the Affected Product Recipient's treating physician's determination that an Affected Product Revision Surgery would be indicated but for a medical condition(s); or
• 180 days after Trial Court Approval

The Green Form and the Orange or Blue Form do not need to be filed simultaneously, but all Forms must be filed within the time limits of the Settlement Agreement.

Yellow Form - Must be postmarked within the time periods prescribed for the Affected Product Recipient's claim for benefits.

Red Form - Must be postmarked no later than 180 days after Trial Court Approval or 180 days after an Affected Product Revision Surgery.
